
In 2020, our clinical team had to adapt clinical services to make sure they remained safe and covid secure. With the temporary closure of the Day Therapy Centres, the change presented an opportunity to develop a new Hospice at Home service in the county of Conwy
Our Hospice at Home service was launched to continue to respond to the needs of our patients by taking care into their homes whilst Day Therapy services are on hold.
The Hospice at Home service is patient-led care and tailored to the individual, replicating the specialist palliative care at the Hospice’s Inpatient Units.
Co-ordinated by Clinical Nurse Specialist (CNS), Helen Russell, Hospice at Home is a ‘hands on’ service in terms of attending to patients’ physical needs, personal care requirements, symptom management, medication administration, general nursing needs, allied palliative care support (including Physiotherapist, Occupational Therapists, Social Worker and Complementary Therapist) and family support.

The Hospice clinical team are working in partnership with District Nurses and GPs to provide a holistic care-plan to those whose preferred place of care and death is at home, and not a hospice or hospital setting.
