INPATIENT UNIT

The clinical team is available to help with symptom control, pain management, end of life care, respite, psychological and emotional support and to deal with any concerns you may have. We also offer support to families and carers. The team communicates with your GP/Consultant, Community Specialist Palliative Care Nurse, Marie Curie, Hospice at Home and Community Nurse to make sure that we have all the relevant information to care for you properly.

A Multi-Disciplinary Team Meeting will be held weekly to discuss your care-plan with all the professionals involved. If you have concerns about the sharing of your information then please let us know.

As a Specialist Unit, we have weekly input from Consultants in Palliative Care and Speciality Doctors who are available throughout the week. St David’s Hospice benefits from having a full multi-disciplinary team. Our nursing team provides a 24-hour service to inpatients and a 10am until 3.30pm, Monday to Friday service in the Day Therapy unit. Our dedicated team has specialist qualifications and a wealth of experience.

Who’s Who

We have a team of experienced, trained nurses who will be caring for you 24 hours a day. The nurse in charge of your care on each shift will introduce him/herself to you, and your carers/family are welcome to discuss any care issues with them with your permission.

An experienced team of Health Care Support Workers also ably assist the trained nurses. They can help you with meals, drinks and hygiene needs and certain nursing tasks, for example, taking your temperature and pulse, under guidance of a trained member of staff.

DAY THERAPY CENTRE

The ethos of the St David’s Hospice Therapy Centre is to enable those with life limiting illness to live well and to live fully. To discover new talents, rediscover old talents and interests; experience various forms of relaxation, alongside a personalised activity program to suit all abilities, all with the support of the Therapy Team.

Complementary Therapy

Such as massage, aromatherapy massage, reiki, reflexology and gentle Indian head massage. Complementary Therapy is beneficial for the management of symptoms such as anxiety, breathlessness and pain

Diversional Therapy

The benefits of Diversional Therapy are to retain skills, knowledge and interests (or discover new ones!). It provides physical exercise, enables socialisation and formation of friendships through shared experiences. It can stimulate the senses, provide expression of feelings, and enhance spirituality, dignity, respect and self-worth through the art of creativity and pleasure

Symptom Management

Ongoing management of any symptoms you may be experiencing will be available Mon-Fri with access to the Advanced Nurse Practitioner/ Clinical Nurse Specialist or Speciality Doctor.

All clinical reviews will be communicated to the health professionals currently involved with your care, (e.g.GP, District Nurse, Macmillan nurse) with your consent.

Occupational Therapist

You can be referred to the Occupational Therapist for an assessment of functional difficulties, such as getting in/out of your home, bathing and/or difficulties managing around the home, enabling you to remain independent within your home.

Physiotherapy

A personalised exercise programme will be developed during an initial assessment with the Physiotherapist. You will have access to a fully equipped therapy gym including; Motomed, treadmill, Bobath plinth and parallel bars

Group Exercise

Such as group indoor tennis, bowling and various ball games, as well as the use of the Nintendo Wii, so you can get as competitive as you want without leaving your chair!

Family Support Service

This includes one-to-one counselling, family counselling and pre-bereavement counselling.

Breathlessness Management

An individualised management plan will be developed by the Physiotherapist to enable you to learn the skills to manage your breathlessness.

 

Garden Project

A form of Diversional Therapy through the medium of gardening.

Accessible to all including wheelchair access, the court yard area will be covered so it can be accessed come rain or shine!

Personalised Non-touch Relaxation Therapy

A non-touch therapy that utilises the body’s energy fields to aid relaxation and a sense of wellbeing.

Chaplaincy

The Hospice Chaplain can be available if you wish to discuss any spiritual or religious views, concerns or questions

Fatigue, Anxiety, Breathlessness Group

This group session is led by the Physiotherapist and the Occupational Therapist to learn and develop the skills required to manage the fatigue and breathlessness associated with many lung conditions.

Group Talks on Any Subject

This helps to retain old memories, gain new knowledge through group discussions, poetry or storytelling.

Social Worker

The Social Worker provides a professional service to the inpatients, day patients, carers and bereaved families known to the Hospice, plus ongoing support to patients, their families and carers.

Assisting in care at home through liaising with community care coordinators or arranging care packages when patients are discharged. The Social Worker will also assist with sourcing appropriate finance for home care and nursing home care where available and appropriate.
